Design requirement: Health data and health-related inference should remain under the jurisdiction of the populations from which they originate.
Implementation guidance:
• Clinical data must be stored and processed within the legal boundaries of the country where it is generated
• Inference generated from that data should be subject to local law, oversight, and audit
• Governments should retain authority over how national health data is used, shared, or licensed
• International organizations should prioritize capacity-building for local compute, secure data centers, and regulatory expertise rather than dependency on external platforms
Operational purpose: Local control ensures that health AI strengthens national health systems rather than extracting value from them. When inference is generated and governed domestically, accountability becomes enforceable, and public trust is preserved.
Precedent:
Countries such as Nigeria and Rwanda have already established data localization and digital health governance frameworks that require clinical data to remain within national borders. These policies demonstrate that sovereignty over health intelligence is achievable without halting innovation.

Long wait times, clinician shortages, cost barriers, and geographic exclusion have created a diagnostic vacuum in which AI systems function as substitutes for the absence of care. In that vacuum, patients do not choose AI freely; they turn to it because the alternative is delay, deterioration, or silence.
That scarcity is not merely a social failure. It has become an economic input.
Desperation converts unmet need into disclosure. Disclosure enables inference. Inference generates value. This is the core mechanism of the emerging health AI economy. Artificial intelligence in healthcare can remain an augmentative public good, extending access, supporting clinicians, and improving outcomes, but only if it is bounded by clear accountability, transparent inference, and the preservation of human judgment. Without those constraints, it will harden into opaque cognitive infrastructure that shapes lives without recourse, consent, or correction.
The Cognitive Revolution is, at its core, not about machines becoming more capable. It concerns institutions’ decisions about whether to remain responsible.
